In order to be medically cleared for athletics at S.S. Seward, the following procedure must be followed. Failure to follow this procedure will result in the student-athlete not being cleared for practice.

1. A physical must have been completed by a licensed health care provider during the 12-month period preceding the beginning of the sport season and a copy must be provided to the school nurse. The sports season includes tryouts. A physical will be valid until one year from the last day of the month during which the physical was performed. For example, a physical performed on August 1,  would be valid until August 31 of the following year.
2. If the physical was completed more than 30 days prior to the beginning of the sports season, an Interval Health History form must be completed and signed by a parent or guardian. The purpose of the interval health history is to ensure that any health problems occurring since the last physical are identified and considered. Students participating in more than one sport will need an interval health history completed for each sport. All immunizations must be up to date.
3. If the 12-month period for the physical expires during a sports season, participants may complete the season, but will need a new physical before participating in another sport.

Exams by personal physicians are encouraged. If this is not possible or practical, the district health care provider will conduct examinations at school at the request of a parent/guardian but we cannot guarantee that physicals will be completed in time for your student to participate in a particular sport. Physicals are also offered on select dates throughout the summer and are posted on the school website toward the end of each school year.
