Beginning July 1, 2018, all New York State public school students must have a health exam when they enter school as a new entrant and in pre-k or kindergarten and grades 1, 3, 5, 7, 9, and 11.
These examinations must be completed on the approved NYSED Student Health Examination Form for School for the health exam.
Students who participate in sports need yearly physicals. In addition, a current physical (completed within 12 months) is also needed when you apply for working papers.
Parents/guardians may opt to have physicals completed by Horizon Family Medical, the district’s healthcare provider. Dates are scheduled during each summer as well as during the school year. Please contact the school nurse for more information.
